What to bring to your first appointment
For your first appointment, please bring:
- Your doctor’s name and phone number
- Your medical and medication history
- Your dental health insurance card and policy booklet
Financial policy
Payment is due at the time of treatment. We offer payment plans in some circumstances.
Insurance
We’ll work with you to determine your coverage and build your treatment plan. You are responsible for the full cost of your treatment, which includes any amount not covered by your insurance.
